Chronic Care Management

Medicare patients with two or more chronic health conditions can benefit from Fort Madison Community Hospital’s Chronic Care Management Program.

Chronic care management is a partnership between patients and the hospital’s health care team. Together, they develop a personalized care plan that includes managing conditions, coordinating resources and removing barriers “to help patients obtain their health care goals.”

Barriers to care include medication costs, lack of support services including transportation, and not understanding health information or how the health care system works.

Who is eligible for chronic care management?

Chronic care management is available to patients enrolled in Medicare Part B who have two or more chronic conditions, such as:

  • Arthritis
  • Asthma
  • Atrial fibrillation
  • Cancer
  • Cardiovascular disease
  • Chronic obstructive pulmonary disease (COPD)
  • Depression
  • Diabetes
  • Hypertension (high blood pressure)

What to expect?

  • Your primary care provider can help you determine if chronic care management is a good choice for you. You can enroll in the program at one of your regularly scheduled visits, including our Annual Medicare Wellness Visit.
  • A comprehensive care plan that addresses your needs will be developed and maintained during your participation in the Chronic Care Management Program.
  • Regular check-ins with your care coordinator to review progress, update your care plan, coordinate care in the health care system and with community resources, and answer questions about your health care.

Does it cost to enroll in Chronic Care Management?

The average monthly co-payment for chronic care management services depends on your coverage and time spent coordinating your care. Participants with secondary insurance or Medicaid often don’t have any out of pocket expenses for these services. To confirm your shared cost, call the customer service telephone number on your insurance card. Fort Madison Community Hospital will submit monthly claims to Medicare and secondary insurance providers. You can discontinue chronic care management at any time.
